ACT Team Nurse - Lewiston (95)

GENERAL SUMMARY:

Under the direction of a Program Clinical Supervisor and in conjunction with a designated ACT Team Medication Provider, the ACT Team Nurse is responsible for crisis assessment, physical health management, and medication management for consumers in the ACT program. The ACT Team Nurse works closely with ACT providers regarding consumers' treatment and medication management.

ESSENTIAL JOB FUNCTIONS:*

Demonstrates commitment to the mission of the agency with a strong understanding and respect for the goals of agency activities.

Provides counseling and/or therapy according to a plan of care development from assessment and diagnostic evaluation.  Assesses changes in the condition of the consumer and modifies the plan of care as appropriate.

Develops a written Individual Support Plan in conjunction with the consumer, the consumer's family where appropriate, the ACT Team, and significant others in accordance with consumer/family needs and in compliance with agency procedures.  Adheres to Medicaid and Medicare regulations for plans of care, when appropriate.

Monitors and assesses weight, BMI, blood pressure, overall physical health for ACT Team consumers.

Provides therapeutic treatments that are both clinically sound and cost effective.  Carries out therapeutic treatments and procedures according to established care plans, agency and program policies and procedures, professional standards, and within the scope of practice of his/her professional license.  Refers consumers to other professionals or services as appropriate to ensure optimal service.

Assists consumers in developing the knowledge and skills and acquiring the necessary supports to live as independently as possible in the least restrictive setting in the community.  This includes: budgeting/money management, hygiene, housekeeping, nutrition, shopping, representative payeeships, medication adherence, physical health as needed and community integration activities.

Ensures that consumers understand their rights regarding confidentiality and that the rights of consumers and significant others are protected as delineated in applicable standards, policies, and procedures

Utilizes the consumer's electronic health record (EHR) to ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.

Supports the consumer's right to make decisions related to his/her plan of care.

Follows policies and procedures with regard to the EHR to ensure that the services provided are consistent with best practices, assessment and/or treatment goals.

Provides family education and consultation, if desired, by consumers receiving community support services in order to help family members and/or other natural supports to develop support systems and help the person to manage their mental illness. 

Informs the Manager and/or Program Team Lead of any perceived problem areas related to consumers, programs/services, the agency, or the community.

Provides public education and professional consultation services within and outside of the ACT program, when appropriate.

Advocates with internal and external customers on behalf of the consumer.

May use personal vehicle for agency-related business as required.

Advocates and Coordinates care with local pharmacies, health insurances, and medical providers. 

Responsible for prior authorizations, review of lab work, staging prescriptions, ordering refills, medication cadies, medication deliveries/administering/injections, and the overall cleanliness/management of the medication room to include medication samples.

Provides supportive counseling, medication, and psycho-educational services to individuals and others as needed, both on and off site.

Meets established direct service expectations.

Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and consumers.

Provides on-call coverage and works shift assignments which will include some evening hours, weekends, and holidays.

May oversight CRMA duties related to medication preparation for consumers of the ACT Team.

Completes and submits required documentation in a timely manner and according to policies and procedures.

Participates in training to maintain or enhance job skills.

Ensures that confidentiality is maintained at all times in accordance with federal, state, and agency standards.

Adheres to the agency's Risk and Safety Management Programs.

Adheres to the agency's Organizational Standards for Performance (see attached).

Attends meetings and supervisions as required and necessary for the position.

Promotes and fosters effective relations, both inside and outside of the agency.

Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.

May perform other related duties as required.

EDUCATION AND EXPERIENCE REQUIRED:

Must be licensed by the State of Maine as a registered nurse.

Preferred Bachelor's degree in Nursing; Certified American Nurses' Association as a psychiatric and mental health nurse.

Training and experience with community mental health, community support rehabilitation, and recovery and wellness required.

SUPERVISORY RESPONSIBILITY:

This position has no supervisory responsibilities.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Normal office environment, not subject to extremes in temperature, noise, odors, etc.; may also be subjected to unpredictable environments with extremes in temperature, noise, odors, weather conditions, etc.

Regular use of computer keyboards, telephone and operating office machines, requiring eye-hand coordination and finger dexterity. 

Occasional lifting and carrying of supplies, files, documents, records, etc.

Required to travel in personal vehicle in all types of weather.
