The Case Manager (Health Home Coordinator) is responsible for assisting persons with serious and persistent mental illness by providing rehabilitative and treatment services as well as environmental supports that promote the integration of the client into the community, sustain the client in their current living situation or another living situation of their choice, and enhance the quality of their life.
