ESSENTIAL JOB FUNCTIONS:

  • Provides counseling and/or therapy according to a plan of care developed from assessment and diagnostic evaluation.
  • Evaluates and assesses the holistic needs of the individual client.
  • Develops a written plan of care in conjunction with the client, the client's family where appropriate, and significant others in accordance with client/family needs and in compliance with agency procedures.  Adheres to Medicaid and Medicare regulations for plans of care, when appropriate.
  • Supports the client's right to make decisions related to their plan of care and ensures that clients understand their rights regarding confidentiality and that the rights of clients and significant others are protected as delineated in applicable standards, policies, and procedures
  • Utilizes the electronic health record (EHR) to ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.
  • Provides therapeutic treatments that are both clinically sound and cost effective and carries out therapeutic treatments and procedures according to established care plans, agency and program policies and procedures, professional standards, and within the scope of practice of the employee's professional license.
  • Assesses changes in the condition of the client and modifies the plan of care as appropriate.
  • Refers clients to other professionals or services as appropriate to ensure optimal service.
  • Arranges medication call-ins with various prescribing psychiatrists or nurse practitioners.
  • Discontinues services appropriately, referring for additional follow-up services from the health care community, when warranted.
  • Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and consumers.
  • Develops, maintains and appropriately utilizes knowledge of community resources.
  • Educates and instructs client and significant others in prevention and rehabilitative measure appropriate for the client's care and safety.

EDUCATION AND EXPERIENCE REQUIRED:
  • Duties require a Master's Degree in Social Work, Psychology/Counseling or Psychiatric Nursing.  Three to five years of directly related experience preferred. 
  • Must possess LMSW/LCPC/LMFT conditional clinical license or APRN-CNS clinical licensure with PMHCNS-BC certification.  Clinicians that are dually licensed as a CADC or LADC are preferred.
  • Experience working with and knowledge of co-occurring disorders preferred.
  • Must possess a valid State of Maine driver's license if required to travel.
