GENERAL SUMMARY:

Under the direction of the assigned Program Clinical Supervisor, the CRS Case Manager is responsible for assisting persons with chronic mental health problems by providing rehabilitative and treatment services as well as environmental supports that promote the integration of the consumer into the community, sustain the consumer in his/her current living situation or another living situation of his/her choice, and enhance the quality of his/her life.

ESSENTIAL JOB FUNCTIONS:*

Demonstrates commitment to the mission of the agency with a strong understanding and respect for the goals of agency activities.

Provides community outreach and case management services while assisting consumers in developing the knowledge and skills and acquiring the necessary supports to live as independently as possible in the least restrictive setting in the community.  This includes: budgeting/money management, hygiene, housekeeping, nutrition, shopping, representative payeeships as needed and community integration activities.

Helps consumers identify strengths, set goals and develop a life plan; participates with team members in the development of a holistic assessment of the consumer, including physical, emotional, psychological, and spiritual.

Participates with consumer and other staff in the development and implementation of a plan that supports and advocates for the attainment of consumer's life goals.

Provides supportive counseling and psycho-educational services to individuals and others as needed, both on and off site.

Provides on-call coverage and works shift assignments which will include some after hours, weekends, and holidays.

Provides crisis intervention, assessment, referral, and follow-up in an environment safe both for the consumer and staff.

Collaborates with family members and significant others as appropriate.

Meets established direct service expectations.

Provides family education and consultation, if desired, by a person receiving community support services in order to help family members and/or other natural supports to develop support systems and help the person to manage their mental illness.

Utilizes the consumer's electronic health record (EHR) to ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.

Follows policies and procedures with regard to the EHR to ensure that the services provided are consistent with best practices, assessment and/or treatment goals.

Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and consumers.

Participates in the provision of services in a team approach, networking with other providers and providing backup services in the absence of other staff.

Provides public education and professional consultation services, when appropriate.

Uses personal vehicle as may be required for consumer transportation, outreach in the community, home visits, and/or agency related business.

Locates, uses, monitors, and/or develops community resources and services for consumers.

Engages consumers in developing rehabilitation plans and programming.

Maintains and utilizes knowledge of community resources.

OTHER DUTIES AND RESPONSIBILITIES:

Communicates effectively both verbally and in writing, establishes positive public relations and interacts effectively with a diverse array of professionals and consumers.

Maintains appropriate boundaries and displays respect and courtesy to all internal and external customers.

Completes and submits required documentation in a timely manner and according to policies and procedures.

Participates in training to maintain or enhance job skills.

Ensures that confidentiality is maintained at all times in accordance with federal, state, and agency standards.

Adheres to the agency's Risk and Safety Management Programs.

Adheres to the agency's Organizational Standards for Performance (see attached).

Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.

Attends meetings and supervisions as required and necessary for the position.

Promotes and fosters effective relations, both inside and outside of the agency.

May perform other related duties as required.

EDUCATION AND EXPERIENCE REQUIRED:

Minimum of MHRT/C Provisional certification required.  Demonstrated knowledge in the parameters and needs of adults with severe and persistent mental illness equivalent to completion of a Bachelor's degree or an equivalent combination of education and experience is preferred.

Must possess a valid State of Maine driver's license.

Familiarity with psychotropic medication usage and monitoring its use.

Proficient with the use of a computer and job-specific software applications

WORKING CONDITIONS/PHYSICAL DEMANDS:

Residential environment; may be subjected to unpredictable environments with extremes in noise, odors, etc.

Occasional use of computer keyboards, telephone and operating office machines, requiring eye-hand coordination and finger dexterity. 

May perform tasks that include lifting, bending, stooping, twisting, pulling, pushing, climbing stairs, walking and standing while working with consumers.

Works with adaptive equipment (reachers, dressing devices, etc.).

May be asked to operate a motor vehicle in all types of weather.
