Direct Support Professional - Social Learning Center

GENERAL SUMMARY:

Under the direction of the SLC Program Manager and Manager, the Direct Support Specialist (DSS) is responsible for providing care management and skill instruction to persons with developmental disabilities, including persons with intellectual and developmental disabilities, autism, or multiple functional and/or medical limitations.

ESSENTIAL JOB FUNCTIONS:*

Demonstrates commitment to the mission of the agency with a strong understanding and respect for the goals of agency activities.

Participates as a team member in the evaluation and assessment of the support needs of consumers.

Develops a written Person Centered Plan with the Program Plan Coordinator and in conjunction with Social Learning Center team members, the consumers, their family members and significant others (guardians, representatives, correspondents) where appropriate in accordance with the needs of the consumers and in compliance
with established procedures.

Develops and plans daily activities depending upon the individual needs and desires
of consumers.

Provides small group and one-on-one instruction according to established care plans, agency, and program policies and procedures, and generally accepted standards of care.

Provides program supports for one to ten consumers, including but not limited to:  orienting the consumers to the Center and the agency; advocating whenever necessary on behalf of the consumers; assuring the appropriateness of Person Centered Plan implementation; and coordinating all aspects of the semi-annual review according to established policies and procedures.

Assesses changes in the functioning of the consumers and modifies the plan of care, as appropriate, in conjunction with the consumers and all other interested parties and in accordance with established policies and procedures.

Refers consumers to other professionals or services as appropriate to ensure optimal service

Makes accurate and legible entries in the permanent health record of the consumers according to established policies and procedures, including the writing of progress notes, incident reports, and daily recordings, and gives input into the writing of quarterly and annual reports.

Assists consumers with many levels of their care, including transfers, personal care, pushing or pulling wheelchairs, dressing and undressing, mobility training,
and feeding.

Demonstrates commitment to teaching for independence for consumers and movement towards the least restrictive settings.

Demonstrates ability and willingness to implement positive behavior interventions.

Uses personal or agency vehicle as may be required for consumer transportation, outreach in the community, home visits, and/or agency related business.

OTHER DUTIES AND RESPONSIBILITIES:

Communicates effectively both verbally and in writing, establishes positive public relations and interacts effectively with a diverse array of professionals and consumers.

Maintains appropriate boundaries and displays respect and courtesy to all internal and external customers.

Demonstrates patience, caring, compassion, and sensitivity toward consumers.

Completes and submits required documentation in a timely manner and according to policies and procedures.

Participates in training to maintain or enhance job skills.

Ensures that confidentiality is maintained at all times in accordance with federal, state, and agency standards.

Adheres to the agency Exposure Control, Risk Management and Safety Management Programs.

Adheres to the agency's Organizational Standards for Performance (see attached).

Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.

Attends meetings and supervisions as required and necessary for the position.

Promotes and fosters effective relations, both inside and outside of the agency.

May perform other related duties as required.

ADDENDA 

For Activity Services Program:

Knowledge of speech, physical, occupational, and communication therapy.

Ability to develop age appropriate activities.

For Social Activity Services Program:

Knowledge of the normal aging process.

For Work Activity Services Program:

Knowledge of sheltered employment or work activity.

Knowledge of support skill training curriculum.

For Recreation Coordinator:

Responsible for coordination, development, and supervision of activities for Le Club Bon Amis, a social recreational club for adults with developmental disabilities.

Plans and supervises regular Friday night social gatherings.

Organizes special community outings.

For Housekeeper:

Performs wide range of cleaning duties, using specialized housekeeping equipment and chemicals.

Orders materials and supplies, coordinating vendors and pricing.

Adheres to agency Exposure Control program

For Relief or Per Diem Staff (all SLC Programs):

Availability: must be available for work on an as-needed basis.

Additional responsibilities vary by program, as listed above.

For Home Supports Program:

Works with consumers (and their families) in the home/community.

Provides oversight or full support with medications.

May transport consumers to and/or attend medical appointments.

Communicates with outside agencies and departments for coordination of care.

Attends internal and external meetings as they relate to the home supports program.

Complete daily progress notes and goal tracking sheets.

EDUCATION AND EXPERIENCE REQUIRED:

High School diploma or equivalent required.  Related experience working with persons with acute and severe intellectual and developmental disabilities or experience in adult day programming, health care or social service setting preferred.

Must possess either a DSP or equivalent certification at time of employment, or be eligible to subsequently complete and actively maintain all requirements of the certification within initial six (6) months of employment.

Home Supports positions also require current CRMA certification and six (6) months of related experience.

Must possess a valid State of Maine driver's license.

Depending on the program assignment, duties may also require the following:  Knowledge of speech, physical, occupational and communication therapy; ability to develop age-appropriate activities; knowledge of the normal aging process; knowledge of sheltered employment or work activity; knowledge of support skill training curriculum.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Normal office environment, not subject to extremes in temperature, noise, odors, etc.; may also be subjected to unpredictable environments with extremes in temperature, noise, odors, etc.

Occasional use of computer keyboards, telephone and operating office machines, requiring eye-hand coordination and finger dexterity 

Required to bend at the waist, pivot, and lift using upper body strength

Required to pull, push, and lift while in a standing or kneeling position

Requires pulling, tugging, and twisting at the waist

Requires ability to recognize and assist consumers who are choking, screaming, bolting from the building, or engaging in other dangerous activity

May be asked to operate a motor vehicle in all types of weather
