GENERAL SUMMARY:

Under the direction of a Program Clinical Supervisor, the Clinician is responsible for providing professional therapeutic mental health services and consultation to local agencies and other professionals in the community.

ESSENTIAL JOB FUNCTIONS:*

Demonstrates commitment to the mission of the agency with a strong understanding and respect for the goals of agency activities.

Provides counseling and/or therapy according to a plan of care developed from assessment and diagnostic evaluation.

Evaluates and assesses the holistic needs of the individual consumer.

Develops a written plan of care in conjunction with the consumer, the consumer's family where appropriate, and significant others in accordance with consumer/family needs and in compliance with agency procedures.  Adheres to Medicaid and Medicare regulations for plans of care, when appropriate.

Supports the consumer's right to make decisions related to his/her plan of care.

Ensures that consumers understand their rights regarding confidentiality and that the rights of consumers and significant others are protected as delineated in applicable standards, policies, and procedures

Utilizes the consumer's electronic health record (EHR) to ensure that documentation is complete, accurate, timely and meets regulatory and agency standards.

Follows policies and procedures with regard to the EHR to ensure that the services provided are consistent with best practice, assessment and treatment goals.

Provides therapeutic treatments that are both clinically sound and cost effective.

Carries out therapeutic treatments and procedures according to established care plans, agency and program policies and procedures, professional standards, and within the scope of practice of the employee's professional license.

Assesses changes in the condition of the consumer and modifies the plan of care as appropriate

Refers consumers to other professionals or services as appropriate to ensure optimal service.

Arranges medication call-ins with various prescribing psychiatrists or nurse practitioners.

Discontinues services appropriately, referring for additional follow-up services from the health care community, when warranted.

Arranges for consumer hospitalization, as necessary.

Interacts with other agency programs for referrals, as necessary.

Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and consumers.

Develops, maintains and appropriately utilizes knowledge of community resources.

Provides education, professional consultation services and court testimony as necessary.

Educates and instructs consumers and significant others in prevention and rehabilitative measure appropriate for the consumer's care and safety.

OTHER DUTIES AND RESPONSIBILITIES:

Communicates effectively both verbally and in writing, establishes positive public relations and interacts effectively with a diverse array of professionals and consumers.

Maintains appropriate boundaries and displays respect and courtesy to all internal and external customers.

Demonstrates patience, caring, compassion, and sensitivity toward consumers.

Completes and submits required documentation in a timely manner and according to policies and procedures.

Participates in training to maintain or enhance job skills.

Ensures that confidentiality is maintained at all times in accordance with federal, state, and agency standards.

Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.

Promotes and fosters effective relations, both inside and outside of the agency.

May use personal vehicle for agency-related business as required (essential function for ACT) .

Adheres to the agency's Risk and Safety Management Programs

Adheres to the agency's Organizational Standard for Performance (see attached)

May perform other related duties as required.

ADDENDA

ACT Team:

May provide case management services including: street and community outreach; housing location/stabilization; and insuring the provision and maintenance of basic needs (food, clothing, etc.).

Assists consumers in developing the knowledge and skills and acquiring the necessary supports to live as independently as possible in the least restrictive setting in the community.  This includes: budgeting/money management, hygiene, housekeeping, nutrition, shopping, representative payeeships as needed and community integration activities.

Links to and provides support with other services (vocational, educational, social/recreational, transportation, etc.) and advocates for entitlements such as Mainecare.

Provides interface between the ACT Team and other service providers throughout the state to specific consumers.

Assists consumers in obtaining non-psychiatric medical care.

Provides on-call coverage and works shift assignments which will include some after hours, weekends, and holidays.

Provides education with consumers around symptom management, substance
abuse, etc.

Uses personal vehicle as may be required for consumer transportation, outreach in the community, home visits, and/or agency related business.

For ACT Intake Coordinator:

Screens potential ACT referrals for clinical necessity (via telephone and document review).

Schedules intakes with clients, guardians, or referral source.

Maintains PSARs for all ACT referrals.

Completes intake assessment for ACT (average of two per week), coordinating additional intakes as needed.

Completes intake review at ACT team meeting to determine disposition of intake.

Competes prior authorizations and other required intake paperwork.

Monitors client transition to ACT for first 30 days.

EDUCATION AND EXPERIENCE REQUIRED:

Duties require a Master's Degree in Social Work, Psychology/Counseling or Psychiatric Nursing.  Three to five years of directly related experience preferred. 

Must possess LMSW/LCPC/LMFT conditional clinical license or APRN-CNS clinical licensure with PMHCNS-BC certification.  Clinicians that are dually licensed as a CADC or LADC are preferred.

Experience working with and knowledge of co-occurring disorders preferred.

Must possess a valid State of Maine driver's license if required to travel.

Familiarity with psychotropic medication usage and monitoring its use.

Proficient with the use of a computer and job-specific software applications

Ability to assess strengths, needs, trauma history, and mental health symptoms in a culturally and linguistically appropriate manner. (SOT Clinician)

WORKING CONDITIONS/PHYSICAL DEMANDS:

Normal office environment, not subject to extremes in temperature, noise, odors, etc.

Regular use of computer keyboards, telephone and operating office machines, requiring eye-hand coordination and finger dexterity. 

Occasional lifting and carrying of supplies, files, documents, records, etc.
