PATH Case Manager - Lewiston (422)

GENERAL SUMMARY:

Under the direction of a Program Clinical Supervisor and assigned Manager the PATH Case Manager is responsible for providing assertive and ongoing outreach efforts to identify and engage people who are homeless with a mental health diagnosis,  not receiving services or basic survival supports, for the purposes of informing individuals of community resources, providing referral to temporary safe places to reside, re-establishing relationships with families and /or local communities, assuring timely access to local resources and supports, and obtaining or developing skills needed for independent adulthood.

ESSENTIAL JOB FUNCTIONS:*

Demonstrates commitment to the mission of the agency with a strong understanding and respect for the goals of agency activities.

Provides outreach to literally homeless individuals in area shelters, on the streets, and at other locations

Engage literally homeless individuals and assess eligibility for the PATH program and their strengths, challenges and obstacles to housing and mainstream resources

Arrange for clinical assessment to determine eligibility for the PATH program

Provides linkage to mainstream resources including, but not limited to: housing, income and medical resources, applying for MaineCare and SSI/SSDI benefits and employment services, and assists consumers in accessing and securing services .

Collects, documents and reports consumer information in compliance with agency and contract reporting requirements, including HMIS and/or other homeless data tracking systems.

Maintains current listing of community resources and interfaces with other agencies to provide effective referrals for treatment services.

Assists consumers in obtaining non-psychiatric medical care.

Provides education with consumers around symptom management, substance
use, etc.   

Complete and apply learning of SOAR(SSI/SSDI Outreach, Access and Recovery Model) online training within 6 months of hire.

Collaborates with family members and significant others as appropriate.

Meets established direct service expectations.

Utilizes the consumer's electronic health record (EHR) and HMIS to ensure that documentation is complete, accurate, and timely and meets regulatory and agency standards.

Follows policies and procedures with regard to the EHR to ensure that the services provided are consistent with best practices, assessment and/or treatment goals.

Participates in scheduled meetings, including case reviews, supervisory sessions, staff meetings and other inter-agency and intra-agency meetings as appropriate for program and consumers.

Maintains and utilizes knowledge of community resources.

Conduct outreach in assigned area, including but not limited to leaving PATH program cards, outreach brochures etc. where people who are homeless are known to be.

Uses personal vehicle as may be required for consumer transportation, outreach in the community, home visits, and/or agency related business.

Adheres to the agency's Organizational Standards for Performance.

Functions in a cost-effective manner with regard to the utilization of time, supplies and other agency resources.

Attends meetings and supervisions as required and necessary for the position.

Promotes and fosters effective relations, both inside and outside of the agency.

May perform other related duties as required.

EDUCATION AND EXPERIENCE REQUIRED:

Minimum of MHRT/C Provisional certification required.  Duties also require knowledge of parameters and needs of adults with severe and persistent mental illness equivalent to completion of a Bachelor's degree in a human services and/or an equivalent combination of education and experience.

Must possess a valid State of Maine driver's license.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Normal office environment, not subject to extremes in temperature, noise, odors, etc.;

may also be subjected to unpredictable environments with extremes in temperature, noise, odors, weather conditions, etc.

Regular use of computer keyboards, telephone and operating office machines, requiring eye-hand coordination and finger dexterity. 

Occasional lifting and carrying of supplies, files, documents, records, etc.

May be asked to operate a motor vehicle in all types of weather.
